It is a two part process

Apply for Emblem Use Authorization Statement (EUAS) below and take it with you when you go pick up your specialty plate.
Apply for specialty plate from Missouri Department of Revenue.

Apply for Emblem Use Authorization Statement.

If you are interested in getting a new vanity plate.
If you are interested in renewing your vanity plate.
Please donate $25 for 1 year or $50 for 2 year.
